Executive Physical explained
An executive physical is a full day, or occasionally two, in which a clinic runs through a battery of tests in one visit and hands you a single written report: bloods, imaging, a heart assessment, a fitness test, cancer screening appropriate to your age and sex, and a long consultation to put it together.
What is genuinely being sold is compression and coordination. The individual components are almost all available through ordinary healthcare; getting them done in a day, read by one team, and explained in one sitting is not. For people who otherwise postpone appointments for years, that is the whole value.
Contents vary far more between clinics than the shared name suggests. Two programmes advertised at the same level can differ on whether they include a coronary calcium score, an exercise stress test, a sleep assessment, or a scan of the arteries in the neck. The itemised list is the only way to compare them.
The same caveat applies as to any screening in people without symptoms: the more tests you run, the more ambiguous findings you generate, and each one costs time, money and worry to resolve. A good programme is explicit about which of its tests are established screening and which are extras that come with that trade-off.

Imaging is the single biggest driver. A programme built around blood work and a clinical examination costs a fraction of one built around cross-sectional imaging, and the presence or absence of an MRI or a CT usually explains most of the gap between two quoted prices.
The second driver is physician time. An hour with a doctor at the end, rather than a report in the post, is a substantial part of what the higher-priced programmes charge for.
Whether follow-up is included matters more than it looks. Some programmes include the specialist referral and the repeat scan when something ambiguous appears; others hand you a report and a recommendation you then have to buy.
Corporate schemes often pay for these programmes for senior staff, which is where the name comes from. If yours might, ask before booking privately.
Who it suits, and who it does not
It suits people with no regular doctor and years of skipped check-ups, people who travel constantly and cannot assemble care in one place, and people whose employer is paying.
It suits people less well if they already have a good primary care relationship. Much of the programme duplicates what that doctor can order, often at no cost, and the coordination you are paying for already exists.
It is not a substitute for the national screening programmes you are already invited to, and anyone with an active symptom should be seen for that symptom rather than booking a screening day.

What is included in an executive physical?
Typically a comprehensive blood panel, a physical examination, a heart assessment such as an electrocardiogram or an exercise stress test, a fitness measurement, age-appropriate cancer screening, and a long consultation. Imaging is the component that varies most between clinics.
How long does an executive physical take?
Most run four to eight hours in a single day. Programmes that include cross-sectional imaging and multiple specialist reviews can run over two days.
Is an executive physical worth it?
It depends on what you would otherwise do. For someone who has not seen a doctor in five years it consolidates a lot of neglected care into one day. For someone with an engaged primary care doctor, much of it duplicates care they can already get.
Does insurance cover an executive physical?
Usually not as a package, though individual components may be covered when there is a clinical reason for them. Corporate health schemes are the more common route.
Do I need to fast beforehand?
Almost always, because the day starts with blood work. Clinics generally ask for an overnight fast and give specific instructions about medication and water.
