Biological Age Testing explained
A biological age test estimates how old your body appears to be, as distinct from how many birthdays you have had. Most clinic tests use an epigenetic clock: a laboratory measures chemical tags called methyl groups attached to hundreds of specific sites on your DNA, and an algorithm converts that pattern into a number of years.
There is no single clock and they do not agree. First-generation clocks were trained to predict chronological age; later ones - the family the field calls second and third generation - were trained instead on health outcomes and mortality, which is why two laboratories can hand the same person ages several years apart. The clock matters more than the result, and any honest report says which one it used.
The second thing worth knowing is that repeat measurements on the same person move around. Some of that is real change and some is measurement noise, and a single test cannot separate them. This is the finding our own review of the evidence goes into, and it is the reason nobody serious reacts to one number.
A cheaper family of tests skips DNA entirely and computes an age from ordinary blood markers and blood pressure. These are transparent, reproducible, and about as informative for most people - which says something about how much of the DNA version’s appeal is the technology.

The laboratory work is the floor. What moves the price above it is whether a clinician explains the result, whether the test is bundled with a blood panel, and whether the clinic has an exclusive arrangement with a particular testing company.
Second and third generation clocks cost more than the first-generation ones, and saliva kits cost less than blood draws. Some clinics resell a consumer kit at a markup with a consultation attached; others run their own.
Because the value is in the trend rather than the number, the price that matters is the price of the second and third test. Ask for it upfront.
This is not a diagnostic test and is not covered by insurance or a national health service anywhere in this directory.
Who it suits, and who it does not
It suits people who find a single summary number motivating and who will treat it as a rough direction rather than a verdict, and people who intend to test repeatedly over years rather than once.
It suits people less well if a bad number would worry them, or if they would act on a small change. The measurement noise between repeat tests is large enough that small changes are not reliably real.
Anyone hoping to use it to prove that a supplement or protocol worked should know that this is exactly what the test is currently least able to do.

Biological Age Testing: common questions
Are biological age tests accurate?
They are reproducible enough to be interesting and noisy enough that a single result should not be taken literally. Different clocks give different answers for the same person, and repeat tests on the same sample vary. Our evidence review linked on this page goes through what the research actually shows.
What is an epigenetic clock?
An epigenetic clock is an algorithm that reads the pattern of methyl groups - small chemical tags that switch genes on and off - at hundreds of sites on your DNA, and converts that pattern into an estimated age. Different clocks read different sites and were trained on different outcomes.
Can you lower your biological age?
Reported ages do move, and they move in the expected direction in people who change their sleep, weight, activity and drinking. Whether the number moving means the underlying ageing changed, or only the marker did, is the open question the field has not settled.
Blood test or saliva test - does it matter?
Both are used commercially. Blood is the sample type most published clocks were developed on; saliva kits are cheaper and more convenient. What matters more is using the same sample type and the same laboratory every time you retest.
How often should you retest biological age?
Given how much repeat measurements vary, annually is the shortest interval that makes sense, and several years of readings tell you more than any two do.
