What you will find in the United States
Longevity clinics in the United States operate almost entirely on direct payment, because neither Medicare nor most private insurance covers extensive testing of adults who feel well. That single fact shapes the rest of it: prices are quoted to consumers rather than negotiated with insurers, and longevity medicine is not a recognised speciality, so what sits behind the word varies from one clinic to the next.
What that produces is five recognisably different businesses. Standalone imaging centres sell a whole-body magnetic resonance imaging scan, or a coronary artery calcium score - a computed tomography measure of calcified plaque in the heart arteries - as a one-off purchase with a radiologist's report. Doctor-led concierge and direct primary care practices charge a monthly or annual membership and have added long blood panels, body composition and fitness testing to what was an ordinary general practice.
Several large hospital and academic systems run executive health programmes that compress a day of tests and consultations into one visit, billed directly rather than through insurance. Then there are single-service operations - intravenous infusion rooms, hormone and testosterone clinics, regenerative medicine and stem cell providers - and national telehealth companies where the blood is drawn at a commercial laboratory near you and the clinician reading it sits in another state. The residential medical retreat common in Central Europe is rare here; the American package is usually a day, not a week.
Longevity medicine in the United States
The United States has no single public health system, and its largest public payer buys prevention from a list. Medicare, the federal programme for people aged 65 and over and for some younger disabled people, covers preventive services only where a statute, a regulation or a national coverage determination specifically authorises them. It does not pay for a routine physical examination: the yearly Wellness visit it does cover is a health risk questionnaire and a conversation that produces a personalised prevention plan, and its cardiovascular screening benefit is a cholesterol, lipid and triglyceride blood test once every five years. A physician can also leave the programme altogether, filing a two-year affidavit to opt out and contracting privately with patients instead, which is one way concierge practices are arranged.
For everyone else the floor is section 2713 of the Public Health Service Act, which requires most private plans to cover, without cost sharing, whatever the United States Preventive Services Task Force - an independent panel supported by the Agency for Healthcare Research and Quality - grades A or B. The Supreme Court upheld the way those panel members are appointed in June 2025, leaving the requirement standing. The list is short on purpose: the Task Force assesses only what is worth doing to people with no signs or symptoms, it grades a service D when it recommends against it, and it issues an I statement when the evidence is insufficient to judge.
Several of the tests sold here have an ordinary route when there is a clinical reason for them. The Task Force grades the following A or B, which is why most private plans cover them without cost sharing:
- colorectal cancer screening for adults aged 45 to 75
- annual low-dose chest computed tomography for adults aged 50 to 80 with a 20 pack-year smoking history, roughly a pack a day for 20 years, who still smoke or stopped within the past 15 years
- a single ultrasound for abdominal aortic aneurysm in men aged 65 to 75 who have ever smoked
- bone density scanning for women from 65, and blood pressure measurement for every adult
- blood sugar testing for adults aged 35 to 70 who carry excess weight
A family history or a symptom, described to a physician, often turns a purchase into covered care.
The layer these clinics are built on is a different matter. The Task Force examined adding a coronary artery calcium score, a high-sensitivity C-reactive protein test measuring low-grade inflammation, or an ankle-brachial index comparing blood pressure at the ankle and the arm to standard cardiovascular risk assessment, and concluded that the evidence is insufficient to say whether any of them prevents heart attacks or strokes.
On whole-body computed tomography the Food and Drug Administration is blunter: it knows of no scientific evidence that scanning people without symptoms does more good than harm, and it bars manufacturers of computed tomography systems from promoting them for that use, though it does not regulate the practitioners who buy the machines. That statement addresses computed tomography, which uses X-rays, and does not extend to magnetic resonance imaging, which does not. Neither scan appears among the graded services a plan must cover.
There is no national medical licence. Each state and territory licenses and disciplines its own physicians through its own board, and the Federation of State Medical Boards runs a free public lookup called DocInfo, built from those boards' data, showing whether a doctor holds a licence in good standing and whether disciplinary action has been taken. Board certification is a separate and narrower thing: the 24 member boards of the American Board of Medical Specialties certify physicians in 38 specialities and 89 subspecialities, and longevity or age-management medicine is not among them. Certificates in those fields are issued by private organisations outside that system.
Several federal rules bear on what a clinic can lawfully offer. Testosterone and the other anabolic steroids sit in Schedule III of the Controlled Substances Act, so a prescription requires a clinician registered with the Drug Enforcement Administration. Human growth hormone goes further still: federal law makes it a criminal offence to distribute it for any use other than a disease or recognised medical condition authorised by the Secretary of Health and Human Services and prescribed by a physician.
Peptides sit in a regulatory gap. A pharmacy compounding under section 503A of the Federal Food, Drug, and Cosmetic Act may only use bulk substances that meet a United States Pharmacopeia monograph, are components of an approved drug, or appear on the Food and Drug Administration's 503A list. Ipamorelin acetate and the growth hormone releasing peptides GHRP-2 and GHRP-6 sit in category 2 of the agency's interim compounding policies, meaning it has identified potential significant safety risks, in their case under the policy for outsourcing facilities that compound in bulk. BPC-157, CJC-1295, epitalon and MOTS-c appear on the list of nominations their sponsors later withdrew, with the agency recording concerns about immune reactions to the peptide and about impurities left by manufacturing.
For cell therapies the position is narrower again. The only stem cell products approved in the United States are blood-forming cells from umbilical cord blood, for disorders of the blood system, and there are no approved exosome products at all. Anything more than minimally manipulated, or used for something other than its original function in the body, needs approval before it can lawfully be sold.

American insurance does not pay to screen people who feel well beyond those defined lists, and clinics price accordingly. Some will offer to run part of a panel through your plan, which requires a diagnosis code that justifies the test; if the coding does not hold up, the claim comes back to you.
Two pre-tax accounts do apply, a health savings account and a flexible spending account, and since January 2026 a health savings account can also pay direct primary care membership fees - up to $150 a month for an individual, or $300 where the arrangement covers more than one person.
The gap between two quoted prices is rarely the machine. It is whether a physician's time is included or you are buying a scan and a report; whether a board-certified radiologist reads the images and sends that report to your own doctor; whether contrast, repeat imaging and the follow-up conversation sit inside the price or outside it; and whether the fee is a membership that assumes you will come back next year.
Before paying, ask for the estimate in writing, and know that you are entitled to one. Under the No Surprises Act, providers and facilities must give uninsured or self-pay patients a good faith estimate of expected charges when a service is scheduled or on request, with a dispute process if the final bill exceeds it by at least $400.
Most travel for this is domestic, a flight to another state rather than another country, and the licensing question travels with you. Licensure is a state matter, so ask whether the clinician who interprets your results holds a licence where you live - and note that the federal allowance for prescribing controlled substances by telemedicine without an in-person visit is temporary, running to 31 December 2026.
Ask who holds the records, too. Federal privacy law gives you a right to a copy of your own health information, generally within 30 days, so request the images and the raw laboratory values rather than the summary a clinic prints for you.
