VO2 Max Testing explained
A VO2 max test measures the most oxygen your body can take in and use during hard exercise, expressed as millilitres of oxygen per kilogram of body weight per minute. You run on a treadmill or ride a bike while the workload increases every minute or two, breathing through a mask that measures the air going in and out, until you cannot continue.
Clinics offer it because cardiorespiratory fitness measured this way is one of the strongest predictors of long-term health outcomes in large population studies - stronger than most single blood markers. Our own review of what the number actually means by age and sex is linked below.
A laboratory test is a genuinely different measurement from the VO2 max a watch reports. The watch infers a number from your heart rate and pace; the laboratory measures the gas you breathe out. The two can disagree by a wide margin, and only the measured one is a measurement.
The same test usually produces two extra numbers worth more than the headline: the heart rate and pace at your first ventilatory threshold, the point where breathing starts to climb faster than effort, and at the second, where it climbs steeply again. Those two points are what turn a test into a training plan, because they define the intensity bands people mean by easy and hard.

What you are buying is roughly half equipment and half interpretation. A test on a metabolic cart with a technician and a printout sits at one end. Add a physician present for the electrocardiogram, a full report with training zones, and a follow-up conversation, and it costs more.
Adding a twelve-lead electrocardiogram - a recording of the heart’s electrical activity from twelve angles, supervised by a doctor - turns the test into a cardiopulmonary exercise test and moves it into a clinical price band. That version is the one an insurer might cover if it is investigating a symptom.
University exercise-science departments and endurance-sport laboratories run the same protocol for less than most private clinics, without the medical supervision. If you want the number and the training zones, they are worth checking.
Retests are the point, so ask about the price of the second test before you buy the first.
Who it suits, and who it does not
It suits people who train and want their zones set from measurement rather than a formula, and people who want a hard baseline for cardiorespiratory fitness they intend to move over years.
It suits people less well if they are not going to train afterwards. The test is a maximal effort and it is unpleasant; the number by itself changes nothing.
It is a maximal exercise test, so clinics screen for heart disease, uncontrolled blood pressure and recent illness before running it, and some will require medical clearance or insist on a physician-supervised version. Anyone with a known heart condition should be tested in a clinical setting rather than a gym.

What is a good VO2 max?
It depends entirely on age and sex, and a value that is unremarkable for a 25-year-old man can be excellent for a 60-year-old woman. We have a full breakdown by age and sex in the evidence review linked on this page.
How long does a VO2 max test take?
The maximal effort itself is usually eight to fifteen minutes. Allow an hour for the appointment, including fitting the mask, a warm-up, recovery and the explanation afterwards.
Is a watch VO2 max accurate?
It is an estimate from heart rate and pace, not a measurement of the oxygen you use, and it can differ substantially from a laboratory result in either direction. It is useful for tracking your own trend, and unreliable as an absolute number.
How should I prepare for a VO2 max test?
Clinics generally ask for no hard training in the day or two before, no heavy meal in the two to three hours before, no caffeine close to the test, and normal hydration - and then for the same conditions to be repeated at every retest.
What is the difference between a VO2 max test and a cardiopulmonary exercise test?
They are the same exercise protocol. A cardiopulmonary exercise test adds continuous electrocardiogram monitoring and a doctor watching it, which makes it a clinical investigation rather than a fitness measurement.
How often should you retest?
Change takes months, so most people retest every six to twelve months. Testing more often mostly measures how you slept and what you ate.
