Stem Cell Therapy explained
Stem cell therapy in a longevity clinic usually means one of two things: cells taken from your own fat or bone marrow, processed, and injected back - most often into a painful joint - or a product derived from donated umbilical cord tissue. A third category, exosomes, is not cells at all but the tiny vesicles cells release to signal to each other.
The regulatory position is the first thing to establish, because it varies enormously. A small number of stem cell treatments are approved for specific blood and immune conditions. Almost everything offered commercially for joints, ageing or general wellness is not approved, and in several countries it is offered under exemptions that are actively contested by the regulator.
The evidence follows the same pattern. For knee osteoarthritis there are real trials, and they are mixed rather than negative: some show pain improvement, comparisons against a properly blinded placebo injection are less convincing, and there is no good evidence of cartilage being regrown. For systemic anti-ageing use there is essentially no controlled human evidence at all.
This is also the service where medical tourism concentrates, and where the gap between clinics is widest. The same words on two websites can describe a regulated trial-adjacent procedure and an unapproved infusion in a jurisdiction chosen for its permissiveness.

The source of the cells drives the price more than anything else. Cells harvested from you and processed in the same session are priced differently from a donor-derived product that must be bought in, and exosome products differently again.
Multi-session packages and travel packages are common, and both make the underlying per-treatment price hard to compare. Ask for the price of one treatment.
Imaging guidance matters. An injection placed under ultrasound or fluoroscopy by someone trained to do it is a different procedure from a landmark injection, and it costs more for good reason.
None of this is covered by insurance outside the small number of approved indications, and a clinic that offers to help you claim for it is a warning rather than a service.
Who it suits, and who it does not
Most people who come to it have joint pain that has not responded to the usual measures and want to try something before surgery. That is a legitimate reason to have the conversation, with realistic expectations about what the trials show.
It suits people badly if they are expecting systemic rejuvenation. There is no controlled human evidence supporting that use, and it is where the highest prices and the weakest claims sit.
Anyone with an active cancer, an infection, or on immune-suppressing treatment needs a specialist conversation first, not a clinic consultation.

Does stem cell therapy work for knees?
Trials in knee osteoarthritis exist and are mixed. It is a reasonable thing to discuss with a specialist and not a settled treatment.
Is stem cell therapy approved?
A small number of stem cell treatments are approved for specific blood and immune conditions. The great majority of what is sold commercially for joints, ageing or wellness is not approved, and the rules differ substantially between countries.
What are exosomes?
Exosomes are tiny vesicles that cells release to carry signals to other cells. They are not stem cells, and products based on them are regulated differently and studied less. Clinics sometimes present the two as interchangeable; they are not.
