PRP & Regenerative Injections explained

Platelet-rich plasma treatment starts with your own blood. A small draw is spun in a centrifuge to concentrate the platelets - the blood cells that arrive first at an injury and release the signals that start repair - and the concentrate is injected into a painful joint, tendon or the scalp.

Because it is your own blood, the immune risk is minimal and the main complication is the ordinary one for any injection: a sore joint for a few days, and rarely infection. That safety profile is a large part of why it spread so quickly.

The evidence is genuinely mixed rather than absent, and it differs by condition. Knee osteoarthritis and tennis elbow have the most trials, with several showing pain improvement over months and the better-blinded comparisons showing smaller differences.

Hair loss has a growing literature. For many other advertised uses there is little.

A large part of why trials disagree is that "PRP" is not one thing. Preparations differ in platelet concentration, in whether white blood cells are included, in how the blood is spun and in how many injections are given, and two clinics using the same word can be delivering quite different products.

Price tracks the preparation system and the injector. A closed commercial kit that produces a consistent concentration costs more per treatment than a basic centrifuge protocol, and a musculoskeletal specialist injecting under ultrasound costs more than a landmark injection.

Protocols are usually a series - commonly three injections a few weeks apart - so the course price is the real number.

Image guidance is worth paying for in a joint. Placing the injection where it is meant to go is not something to leave to feel.

Insurance rarely covers it, since most authorities still classify it as investigational for musculoskeletal use.

Who it suits, and who it does not

The people with the best case are those with knee osteoarthritis or a stubborn tendon problem that has not settled with rehabilitation, who want an option before surgery and understand the evidence is mixed.

It suits people less well as a first move. Structured rehabilitation, load management and weight change do more for most tendon and joint pain than any injection, and they are cheaper.

It is not appropriate during an active infection, in people with certain blood or platelet disorders, or for anyone whose expectation is that cartilage will be regrown - the trials do not show that.

What is PRP therapy?

It is an injection of your own blood platelets, concentrated in a centrifuge, into a painful joint, tendon or the scalp. Platelets release the signalling proteins involved in tissue repair, which is the rationale.

Does PRP work for knee arthritis?

Several trials report pain improvement over months, and the better-blinded comparisons show smaller differences. It is a reasonable option to discuss for pain that has not settled with rehabilitation, and it is not established as disease-modifying.

How many PRP injections do you need?

Most protocols use a series of around three, spaced a few weeks apart, though this varies by condition and by clinic. The course, not the single injection, is what to price.

Is PRP painful?

The injection itself is comparable to other joint injections, and the joint is commonly sore for two to five days afterwards as the inflammatory response starts. Clinics usually advise avoiding anti-inflammatory painkillers around the treatment.

Is PRP covered by insurance?

Usually not. Most insurers and health services still classify it as investigational for musculoskeletal conditions, so it is generally paid for privately.
