Continuous Glucose Monitoring explained

A continuous glucose monitor is a coin-sized sensor worn on the back of the upper arm for one to two weeks. A filament sits just under the skin and measures glucose in the fluid between cells every few minutes, streaming the readings to a phone. It was built for people with diabetes; longevity clinics offer it to people without.

What it shows that a fasting blood test cannot is the shape of your day. A single morning number is one point; a monitor draws the curve, including the rise after each meal, how long it takes to come back down, and what happens overnight, after exercise, and after a bad night’s sleep.

The practical value is that the response is personal. Two people eating the same meal can produce quite different curves, and the only reliable way to find out which foods do what to you is to look. Most people learn what they needed in the first two to four weeks, which is why clinics sell it as a short programme rather than a permanent habit.

Two cautions worth stating plainly. The sensor reads the fluid between cells rather than blood, so it lags a real blood reading by several minutes and is least accurate when glucose is moving fast. And in people without diabetes, a spike after a meal is normal physiology - there is no established target range for a healthy person, and treating every rise as damage is a good way to end up eating worse rather than better.

Two things are being sold and they are worth separating: the sensors, which are a consumable with a fairly stable market price, and the programme around them - the app, the food logging, and the dietitian or clinician who reads the data with you.

Programmes that include a professional review cost several times a bare sensor. Whether that is worth it depends entirely on whether you would otherwise interpret two weeks of curves on your own.

In some countries sensors are prescription-only, in others they are sold over the counter, and that alone changes the price and how you buy them.

Insurance and national health services generally cover monitors for people with diabetes, and generally do not cover them for anyone else.

Who it suits, and who it does not

It is most useful for people who have been told their blood sugar is drifting up, people with a family history of type 2 diabetes, and people who want to find out how their own body handles particular meals rather than reading a general rule.

It suits people less well if they have a history of disordered eating. A device that scores every meal in real time can make an unhealthy relationship with food considerably worse, and several clinicians will decline to fit one for that reason.

It is unnecessary for anyone already diagnosed with diabetes and managed by a doctor - that is a clinical decision made with the doctor, not a wellness purchase.

Can you use a continuous glucose monitor without diabetes?

Yes, and it is now a common part of longevity clinic programmes. Availability differs by country: in some places sensors are prescription-only, in others they can be bought directly.

How long do you wear a CGM?

Individual sensors typically last ten to fifteen days. Most clinic programmes run two to four weeks in total, which is usually enough to identify personal patterns.

Does putting on a sensor hurt?

Applying it is a brief pinch. The filament that sits under the skin is very fine, and most people stop noticing the sensor within an hour of wearing it.

Are glucose spikes after meals bad?

A rise after eating is normal in everyone, including people in excellent metabolic health. What clinicians look at is how high, how long, and whether the pattern is consistent - not whether a rise happened. There is no agreed target range for people without diabetes.

How accurate is a CGM compared with a finger-prick test?

It measures glucose in the fluid between cells rather than in blood, so it lags a blood reading by roughly five to fifteen minutes and is least accurate when levels are changing quickly. For spotting patterns it is more than accurate enough; for a precise single value it is not the right tool.
