DEXA Body Composition Scan explained
A DEXA scan - dual-energy X-ray absorptiometry, a low-dose X-ray that separates tissue into bone, fat and lean mass - is the clinical reference standard for measuring body composition and bone density. You lie still on an open table for six to twelve minutes, fully clothed apart from metal.
The scan returns three things a bathroom scale cannot:
- Lean mass limb by limb, which is how you find out whether an arm or leg is lagging behind.
- Visceral fat - the fat packed around the organs in your abdomen, which behaves very differently from the fat under your skin.
- Bone mineral density, reported against a healthy young adult reference so that thinning bone is caught long before a fracture.
Its value is in the repeat, not the first scan. A single reading tells you where you are on a population chart. Two readings six to twelve months apart tell you whether a training and eating programme is actually adding muscle or only moving scale weight, which is the question most people actually have.
The radiation dose is small - a body-composition scan is in the region of a few hours of ordinary background radiation, far below a chest X-ray - but it is not zero, which is why clinics will not scan during pregnancy and why scanning monthly is not sensible.

The price gap between clinics comes down to what is attached to the scan rather than the scan itself. A raw scan and a printed report sits at the bottom. Add a technician who positions you consistently between visits, a clinician who reads the bone density result against your fracture risk, and a follow-up scan bundled at a discount, and the price climbs.
Body-composition-only scans are cheaper than scans that also report bone density, because the bone report needs clinical interpretation. Some clinics sell them separately; others report both from the same acquisition.
Sports and university labs often run the same hardware for less than a private clinic, without the clinical read. If you want the number and not the consultation, they are worth checking.
Where a DEXA scan is ordered to investigate suspected osteoporosis rather than to track training, it is a normal medical test and may be covered by insurance or a national health service. Screening scans bought privately are not.
Who it suits, and who it does not
It suits anyone changing their training or eating deliberately and wanting to know whether the change is landing on muscle or on fat, and anyone with a reason to watch their bones - a family history of osteoporosis, an early menopause, long-term steroid use, or a fracture that came too easily.
It is less useful as a one-off curiosity. A single scan without a follow-up gives you a number and no direction, and the number on its own rarely changes what a sensible person would do next.
It is not performed in pregnancy, and results are harder to interpret in people with metal implants in the scanned region or with significant fluid retention, since the scan infers tissue type from how the two X-ray energies are absorbed.

How much does a DEXA scan cost?
It varies widely by country and by what is bundled with it. The directory shows the price band each listed clinic sits in, and the honest answer is that the scan itself is cheap while the clinical interpretation around it is what you are paying for.
How accurate is a DEXA scan for body fat?
It is the most precise widely available method, and much more precise than callipers or a bioimpedance scale. Precision matters more than absolute accuracy here: the same person scanned twice on the same machine will get very close readings, which is what makes tracking change possible.
How often should you get a DEXA scan?
For tracking body composition, six to twelve months is the usual interval, because that is roughly how long meaningful change takes. Scanning more often mostly measures hydration and food in the gut rather than tissue.
Is a DEXA scan safe?
The radiation dose from a body-composition scan is very low - lower than a chest X-ray, and comparable to a few hours of the background radiation everybody is exposed to anyway. It is still avoided in pregnancy.
What is visceral fat, and why does DEXA report it separately?
Visceral fat is the fat stored around the organs inside the abdomen, as opposed to the subcutaneous fat under the skin. It is reported separately because it tracks metabolic risk more closely than total body fat does, and because you cannot see it from the outside.
Do I need to fast before a DEXA scan?
Most clinics do not require it, but they do ask you to keep the conditions the same each time - similar time of day, similar hydration, no heavy training immediately before - so that two scans can be compared.
