The evidence comes mainly from physiology reviews rather than trials of cold showers specifically: researchers studying cold water immersion, cold water swimming, and general cold exposure.

Across 7 studies, the risks are consistent: sudden cold exposure triggers a cold shock response - an involuntary gasp and spike in heart rate and blood pressure - that can precipitate a heart attack or drowning, and prolonged exposure can cause hypothermia, a dangerous drop in core body temperature.
