Does alcohol affect REM or deep sleep? Both, in opposite directions

Recommendation
Alcohol before bed reduces REM sleep, the stage where most dreaming happens, while raising deep sleep in the first half of the night and breaking up the second half. A 2025 meta-analysis, which pools the results of many separate studies, combined 27 of them in healthy adults and found REM sleep was already cut at roughly two standard drinks [13]. The nightcap belief holds up only for the falling-asleep part, and in that analysis only at around five drinks [13].
The findings
Alcohol before bed affects REM sleep and deep sleep in opposite directions. REM sleep, the stage where most dreaming happens, starts later and adds up to less over the night, while deep sleep, also called slow-wave sleep and the hardest stage to be woken from, increases early on [13][14].
A 2025 review that pooled 27 studies of healthy adults found REM sleep was already reduced at a low dose, about two standard drinks, and worsened as the dose rose [13]. A standard drink is the unit researchers use so that beer, wine and spirits can be compared. In the same review, falling asleep faster and dropping into deep sleep sooner showed up only at high doses, around five drinks [13].
The two halves of the night behave differently. In a laboratory study of 24 people aged 18 to 21, alcohol raised deep sleep and cut REM in the first half; in the second half the same people spent more time awake in bed and lost deep sleep, with no rebound of REM to make up the earlier shortfall [5].
Three nights in a row brought no adaptation. In a study where 30 adults slept three consecutive nights on alcohol and three on a matched drink with no alcohol in it, alcohol slowed REM at the start of every night and cut total REM on every night, while the total amount of deep sleep per night did not change [2]. Tolerance to the sedative effect itself, though, builds quickly with repeated drinking [11].
The reviews do not fully agree. A 2013 review of studies in healthy volunteers reported faster sleep onset at every dose and no clear REM effect at low doses, where the 2025 pooled analysis found the reverse on both counts [13][14]. They agree on the two central points: the first REM period is pushed later, and total REM falls at moderate and high doses [13][14].
Why it works
Alcohol is a sedative, and it acts on several of the brain's signalling systems at once [1]. The one that matters most for sleep is the GABA-A receptor, the brain's main brake on nerve activity: alcohol makes that brake respond more strongly, which is what brings sleep on faster while blood alcohol is still high [4].
The brake is temporary. The body clears the alcohol over the course of the night, and the systems it was holding down swing back the other way, a small withdrawal in effect. The brain's own alerting chemicals rise, among them orexin, which keeps you awake, and noradrenaline, part of the body's alarm response, and sleep breaks up [4]. A clinical review describes the same second-half pattern from the drinker's side: longer stretches of wakefulness, a racing heart and sweating [3].
Why alcohol shifts the sleep stages the way it does is still not settled. Work going back decades could not pin the effect on the receptor that benzodiazepine sleeping drugs act on, nor on blocking adenosine, the molecule that builds up while you are awake and creates the pressure to sleep [3].
Heavy long-term drinking produces a different picture from the single-night studies: less deep sleep and more REM than normal, a pattern that can last for months or years into abstinence [1].
Limitations
Almost all of this comes from small laboratory studies of healthy people wired up for polysomnography, an overnight recording of brain waves, breathing and eye movements. Nobody drinks or sleeps that way at home.
The evidence does not transfer cleanly to:
- older adults, because the largest pooled analysis of normal sleep, covering 3,577 people aged 5 to 102, shows deep sleep and REM already shrink with age, and the alcohol studies were run mostly in younger adults [15]
- women, because reviewers of alcohol and sleep in women report that sex differences here remain under-studied [9]
- anyone with sleep apnoea, a disorder in which breathing repeatedly stops during the night, which alcohol can make worse [3]
- people with alcohol use disorder, whose sleep differs from the short-term pattern described here and stays disturbed into abstinence [1][4]
The 2025 pooled analysis could not resolve what alcohol does to total sleep time, to the share of time in bed actually spent asleep, or to minutes spent awake after first falling asleep; the studies were too inconsistent [13].
One of the cleanest single-night experiments used 18 to 21 year olds, an age at which the sleeping brain is still developing, and its authors say alcohol's effects on sleep are shaped by that developmental stage [5]. None of these studies ran long enough to say what repeated nights of shortened REM do to health.
Real-world example
Two glasses of wine with dinner sits inside the low-dose band where the 2025 pooled analysis found REM sleep already reduced, and below the roughly five-drink level at which the same analysis found people fell asleep faster [13]. That night is likely to feel ordinary at bedtime and heavier for the first few hours, then patchy towards morning, with less REM across the night whether or not it is noticed [13][5]. If a wearable reports more deep sleep after drinking, that matches what the lab studies find for the first half of the night; it is not evidence the night as a whole went better [14][5].
